Listed below is the Criteria for each type of incident report. Make sure to get all documentation submitted within 48 hours of the issue! Any questions contact Katie by phone or e-mail

Unlocked or broken door? Suspicious person around the club? Any safety concern should be reported here!
All incedent reports should be using the OCFS approved form above!
Minor incident reporting is a tool to help maintain records of small issues regarding staff. This is not a formal write up and will only be used to assist in documentation.
Examples of minor incidents: inapproproate language, negative interactions with a child, disrespect for fellow staff, leaving spaces unattended etc.
